Some players have worn them while fielding and they're mandatory for on field coaches and bat/ball boys.
They're very tight fitting and don't fall off easily.

if you have ever been hit by a line drive then you'll know why they wear them
duh i can see the reason ffs i'm not totally daft 
curious why they wear them but not the infielders. i take it from your not mentioning them, orioles, that it's not mandatory for the base runners, so they do so through choice. thinking of the risks, they'll be looking at the batter just like the infielders, so no difference there. i suppose they might get hit on the head by a throw while running.

Remember, it may be that a runner is concentrating on something other than the batter and so the ball may come his way in a hurry as he runs. Also, if you're going to go in head first, wearing a helmet is an advantage.
ah yes, i'd forgotten about that. didn't realise full body contact was permitted when competing for a base - remember i'm from the much more gentlemanly world of cricket where obstruction is against the law, and more importantly the spirit! these rough ex-colonials are a bit common to me.
Don't forget the bounty system in MBA. If you can smack Prince Fielder on the head while making it look like you were throwing to third base then you win a years supply of Wrigley sugar-free!
maybe to help protect them when stealing ? a low throw from the catcher could hit them in the head when the runner is diving into 2nd base

Base runners wear them and fielders don't because runners are sometimes not looking at the batter / ball. Such as with a full count and two out runners go no matter what or a runner can set of for a steal but the batter hits it runner would not be looking at the batter/ball.
Players (mostly in the minors) have had serious head injuries running bases without helmets.
