Yes of course it's fundamentally different because there is a large amount of immunity built up in the population, so it can't spread nearly as much or as quickly.
It is similar to a pandemic flu like the Hong Kong flu or Spanish flu because they were new strains and there was no immunity, just like now.
As I said, the issue is the rate of spread, not how deadly it is.
