Added a present to the shopping basket twice and didn't notice ... so two of them arrived.
WTF is my gran going to do with TWO vibrating b*tt plugs?
Seriously though .. I just checked, they be right, me be wrong, own stupid mistake. I thought it gave a warning about "already in your basket"?
Maybe they removed it to sell more items by accident.

They give a warning that you've bought things (say) six months ago, but I do not remember a warning that there is more than one in your basket -- though to be fair, they do show you the basket more than once, along with how much it all costs.

Yep. To click away without first checking that the amount of money about to come out of your account is correct, seems pretty careless to me.
I've often, somehow, had more than one of an item. but it's pretty clear and you have several opportunities to correct,
before you hit "Buy Now"......
As another safeguard don't hit "Buy with one click"....
Having placed your order you then get another notification in an Order Acknowledgement where you can quickly amend your order.
If you paid by Pay-Pal, you get yet another acknowledgement in a receipt so no excuse really!
Best advice, otherwise, is not to buy when you are pissed!

In my experience, Amazon are brilliant at correcting any issues.
