if teams finish level on points,how are positions determined?
goal difference? head to head?

The ranking of each team in each group will be determined as follows:
a) greatest number of points obtained in all group matches;
b) goal difference in all group matches;
c) greatest number of goals scored in all group matches.
If two or more teams are equal on the basis of the above three criteria,
their rankings will be determined as follows:
d) greatest number of points obtained in the group matches between
the teams concerned;
e) goal difference resulting from the group matches between the
teams concerned;
f) greater number of goals scored in all group matches between the
teams concerned;
g) drawing of lots by the FIFA Organising Committee.

D, e and f can happen with a two way tie for 2/3rd. It happened in 1990 with Ireland and Holland tied after every group game was drawn bar England beating Egypt. Holland lost and got Germany in the next round.

What odds rule g magically changing to Fifa ranking if Italy's group has 6 draws.

If there is a tie both teams take part in a penalty shoot versus England, no team has ever been eliminated via this process
