Rafa massively there for the taking. If he'd played like this vs Nishikori he'd be gone.

the time is completely on the umpire. The pre-existing injury thing is tricky. The rulebook only states a vague you can not receive treatment for a condition that has not been aggravated during warm-up or the match. How is anyone ever gonna decide that e.g the blister hasn't become bigger, redder and more hurtful during the match?

it's only moderately warm tosay at like 20°C. Not even Nadal sweats like a pig in those conditions
