if Ramos is as underfed as you say (I've never thought about it), isn't it more likely that Ramos fell down? Don't know how windy it is in Santiago though.
it appeared to be before the match as it happened just after Rufin suspend. unless that was late and they were already on court 
donno buut he was going up all the time even he was6-1 in first set as scoreboard showed ramos winning!
