They use a programming technique called API hooking. When you're writing software for Windows, you make calls to standard Windows functions to do things such as draw text to the screen. Knowing the names of these functions, you can manually watch the execution of the poker client looking for calls to these functions drawing names in the results lobby. Once you've found the code that does this, you write a bit of code that hooks into the Windows functions when the client boots up that grabs this info for you.
It's the same basic technique that is used by datamining software, and when you're writing code to hide evidence of any dodgy software you're running - bots, for example.
It's possible to detect API hooking and then in return it's possible to detect someone looking for API hooking etc. so it can become a cat and mouse game, but ultimately if the sites want to stop it (and therefore stop datamining), they can if they get a good enough programmer in.
