Stage Head to Head markets. (TDF)

Struggling to understand how these work. If 2 riders have the same position its a dead heat.
So does this mean there has to be time between the 2 at the finish?
Or is that only after the top 15 come through. The top 15 even though they have the same time, are distinguishable by the sprint points they get.
It's very unclear.
