There is practically zero state capacity to do anything effectively. I don't even blame calamity lammy for that. It's the entire establishment.

There are 6,500 people attached to Whitehall, supposedly the brains behind the framework that currently operates the prison 'service'.

Whitehall is 20 minutes away from Wandsworth prison.

The first instinct of ministers (Tory and Labour) is to defend that system. Lammy is obviously a total incompetent, way out of his depth, who was always going to be exposed by PMQs. Whilst the underlying issues are not his fault directly, he won't do anything tangible to correct them.

It's the way politics works.  When you're not in the driving seat, it's very easy to tell people where they're going wrong (not so easy to actually resolve).  When you are in the driving seat, all you do is blame the last lot.
