I've asked them two questions on the live chat. One was repeated in full. The other was a two part question including a brief question along the lines of the title of this thread. They omitted this sentence and just showed the other sentence!

No you haven't missed it. This is a very good and straight forward question.
They talk about customer confidence - we might all feel a lot more confident if they answer the difficult questions and tell us exactly what happened, and what they have done to try and ensure it doesn't happen again.

I get the impression they don't yet know for certain, hence these tests they're still running. They could say that though.
I think that's spot on. After the first scheduled test they cancelled the next two, but will do more testing tomorrow. That implies they found something that needs to be fixed and tested, and that couldn't be done until tmrw.
They've answered it at the very end. I'm not sure why that wasn't the first answer since it was this that is the reason for the 'chat'.
That's better - they've given some explanation of where they are.
Templeton - fair point. We'll work to make these sessions better in the future.
