I'd like to nominate It's a Motherfecker by Eels. One of my all-time favourites. Very tragic story behind it for those unaware of the song.

"Gloomy Sunday"

A Hungarian song, "Gloomy Sunday" was first recorded in English by Hal Kemp in 1936, with lyrics by Sam M. Lewis, and was recorded the same year by Paul Robeson, with lyrics by Desmond Carter. It became well known throughout much of the English-speaking world after the release of a version by Billie Holiday in 1941. Lewis's lyrics referred to suicide, and the record label described it as the "Hungarian Suicide Song". There is a recurring urban legend that claims that many people committed suicide with this song playing

You can't get much sadder than that.

Lyrically, I would go for Casimir Pulaski Day by Sufjan Stevens. The contrast of the sing a long finale with the narrative which ends with him questioning his faith is very moving. It was the late Mrs Toteboard's favourite as well Cry
However, my nomination for the saddest piece of music ever is the Belgian sound sculptor Kreng's treatment of Chopin's Prelude number 20 in C minor. It was a mournful piece anyway, but getting someone to cry over the top of it makes it even more powerful; http://www.youtube.com/watch?v=kIxkZF-6Iog

I suppose its how you as an individual define 'saddest'.

To me with a very few exceptions (Green Fields Of France, Enola Gay, Gloomy Sunday & perhaps "Will I see you in Heaven" Eric Clapton. The mass majority of songs mentioned are sad in only the fact that anybody bothered to record them and you have to be a total wuss for them to have any emotional effect.

I much prefer the Matt Williams performance, beautifully and immaculately sung live here at the BBC circa 1974. Very impressive how he holds that note for eternity at the end.
