Dementieva suffered a major shoulder injury in 2000, and altered her service motion to reduce the pain she felt when serving.

She moved her ball toss to the right, and added large amounts of slice to her serve.

The combination of the heavy slice and extreme ball toss position meant that she rarely served to an opponent's backhand, making the positioning of her serve easy to read, and it also meant that her serve was much slower than those of many WTA players: her first serve speed frequently averaged 90 mph (144 km/h), and her second serve speed averaged 75 mph (120 km/h).

Her issues with her serve were further compounded by the fact that the altered ball toss made it difficult to control whether the ball would land in the service box, meaning that she had low first-serve percentages and double faulted frequently.

This was most evident in her loss to Anastasia Myskina in the 2004 French Open final, in which she had ten double-faults.[9]

Between 2004 and 2009, Dementieva was the WTA tour leader in double faults for every year but 2008; in 2009, she served 387 double faults. Her serve made major improvements by the end of her career, and, in the 2009 Wimbledon semifinal, her serve peaked at 112 mph (181 km/h), her first serve speed averaged at 104 mph (167 km/h), and her second serve speed averaged 91 mph (148 km/h).

In her final year on the tour, Dementieva's serve peaked at 119 mph (192 km/h) at the Australian Open.
