If he got three code violation, delay of game warnings, then breaking a racquet and getting warned for it (code violation, unsportsmanlike conduct), then that is a default to my knowledge.

Not necessarily. Was he the receiver? Because as the receiver you are also bound by the 25 second rule between serves. The server loses his first serve, but the receiver loses the point from the second time violation onwards. So maybe he got five time violation warnings?
