First of all and most importantly condolances to all concerned. I can't agree with drivers being confused with motorway turn offs, they're as clear as day. I really can't imagine how you would get confused about which way to go.

Find it difficult to see how he got on to the wrong carriageway in that part of the motorway. He also must have been on the wrong side for a while and didn't pull on to the hard shoulder.
